CPR Grant Program

Dear CPR Donation Applicant:

We are so glad that you have decided to apply for CPR Training through our partner EMS Safety from the Hopey’s Heart Foundation. Please note that in order to apply you must be a 501(c)(3) tax-exempt organization or a government entity (including a public school or school district). Through partner, EMS Safety, if granted, your school or institution must submit twelve individuals to receive CPR Certification. The Foundation will make decisions about recipients on a quarterly basis based on information provided in the CPR Donation Application. We will inform all applicants in writing as to whether they have been selected for an award.

If your school or institution is not selected in the quarterly period in which you apply, your application will be considered again for up to an additional three quarterly grant reviews. If your school or institution does not receive an award during that period and is still interested in being considered, please submit a new application. Please note that submitting an application is not a guarantee that your school or institution will be provided with CPR Training from the Foundation through EMS Safety. Only applicants that are contacted to receive CPR Training will be directed to fill out Hopey’s Heart CPR Course Sign-Up Form. If you have not been selected for CPR Training please do not fill out Hopey’s Heart CPR Course Sign-Up Form only those that have been contacted by Hopey’s Heart Foundation.
If your school or institution is selected to receive CPR Training from the Foundation, it will need to sign a CPR Donation Agreement as a condition of the Foundation’s donation and prior to receipt of CPR Training through EMS Safety. As part of the CPR Training your institution agrees to assume full responsibility for all aspects of your CPR program and for compliance with all relevant laws, regulations and standards of care applicable to your school or institution. This application may be signed by any employee at your school or institution. However, the CPR Donation Agreement must be signed by a person who has authority to legally bind the school or institution receiving the CPR Training. Your school or institution will need to return the signed CPR Donation Agreement to the Foundation within 30 days of being notified that your organization has been selected as a recipient, so please plan accordingly.
